  • Assess older adult's cognitive and social needs
  • Evaluate safety conditions of current residence
  • Develop an individualized plan of care with initial services for older client
  • Make referrals to legal, financial, medical and mental health specialists
  • Recommend opportunities for caregiver support and respite services
  • Accompany client to medical appointments
  • Assist in search for alternative housing as need arises
  • Join client and family on tours of short term care (respite) or long term (independent housing, assisted living, nursing home) care facilities
  • Assist in completion of Insurance/Medicaid applications
  • Arrange for placement of care providers to maintain independence in home setting (i.e. home health care, physical or occupational therapy, meals on wheels, adult day care)
  • Assist with move and transition to new residential setting
  • Maintain ongoing relationship with client and family
  • Communicate with out-of-town family members on an ongoing basis
  • Serve as a mediator between client and family members
  • Continually update client's plan of care as needs change
  • Represent family at facility Care plan meetings
  • Assist with "end of life" issues and referrals for hospice services

 

Taking care of an older person can become a full time job. Salston Elder Care helps relieve you of some of that burden. We will assess your aging loved one’s needs and options and evaluate alternative care situations based on their unique requirements and finances.

Helping your loved one accept the need to leave the home they have known for many years is often one of the most difficult tasks. We can act as a mediator between you and your aging loved one to help them accept the situation and make the transition as smoothly and easily as possible.

We'll establish an ongoing relationship with them, attend physicians' visits and act as a liaison between the family, patient and doctor and monitor their care situation. We’ll help you put all the pieces of the puzzle together to give your aging loved one the best care possible, and give you peace of mind.
